5th Grade Registration will be held at the Elementary Schools

For Registration, parents

will need to bring:

1. Birth Certificate

2. Child’s Social Security Card

3. Immunization Record

4. Proof of Residency(utility bill from last 30 days or
 lease with name & current address)

5. Custody papers (if applicable)

All sessions will be held from 3:45-6:00 pm
